Notes on using this form

This form can be downloaded and saved onto your hard disk drive. It can be completed electronically and then printed out and sent with your subscription to the address that is automatically displayed on the form once you complete the top sections indicating type of membership and selecting the branch (if branch membership option is nominated).   To ensure the automated features on the form work see steps below.

The form is a Microsoft Word document containing several automated features stored as word macros which streamline the completion of the form.  When you open the form in Word you may be prompted to enable macros (depending on the security setting you have in Word).  If prompted then click on the  enable macros button to proceed.  If you receive a message advising that macros are prevented from running then in order to use the form you will need to first ensure that you have Microsoft Word set to enable macros.  Steps to do this are:

Click Macro on the Tools menu.

Click on Security in the macros dropdown menu.

Set the Security level to either medium or low.

bullet

Exit Word to make the new setting effective.

(Earlier versions of Word may have slightly different menu options to these - refer to your on line help in Word for how to enable macros).

 

Once you have downloaded the form and providing you are able to open Word to run a macro as outlined above then proceed as follows:

(1) Open the membership form document that you saved when you downloaded it.

(2) Click on either the "Membership Renewal" or "New Member" option

(3) Click on either "Branch Membership" or "Corresponding Membership" option

(4) If you selected Branch Membership then you can select the Branch from the drop down list that appears when you click on the small arrow button that appears next to the "Branch" field.

(5) Next click in the "Annual Subscription" box which, depending on which option you selected in step (3) or (4) above, this will display the subscription amount relating to Corresponding Membership or the Branch you selected.  The name and address of the Secretary for sending the form to will automatically be displayed at the bottom.

(6) fill in the boxes relating to your name and address and other details

(7) print out and send the form to the address shown.
